MEDICAL AUTHORIZATION REQUIRED

If you are under 18 years of age, you must complete a medical authorization form and have it signed by your parent or guardian. You could also download a form from the Grace Community Church website https://gracechurchmariposa.org/. This form MUST be completed and signed by the parent or guardian before or by the first day of Vacation Bible School in order for your child to attend.
